Through their administrative and support services network, Skilled Healthcare provides direct administrative support services to Hospice Care of the West, LLC, along with Hallmark Rehabilitation, 75 skilled nursing facilities and 22 assisted living facility business affiliates throughout 7 states, including California, Nevada, Arizona, Montana, Idaho, New Mexico and Texas.

In this position, you’ll be responsible for:
Hospice RN Case Manager / Full Time - Greater Los Angeles areas
Bilingual Spanish/English preferred
The Hospice Care of the West Nurse is a member of the interdisciplinary team and is the pivotal person in identifying the physical, psychological, social and spiritual needs of the patient and family. Initiates appropriate intervention and support for the patient and family upon admission to Hospice Care of the West and provides a continuously appropriate, comprehensive and responsive plan of care.
Completion of course study as required to acquire state licensure.
Qualified candidates must be currently licensed as a CA Registered Nurse.. Minimum one year experience medical/surgical, hospice and/or home health preferred. Must have reliable transportation with appropriate insurance coverage.
Hospice Nurse / RN / Per Diem
Bilingual Spanish/English preferred
Enjoys working a flexible schedule noon – 8 pm (Tues through Saturday preferred) in the Greater Los Angeles areas doing follow up patient visits, hospice admissions and RN case management as the need arises. We are willing to discuss scheduling options for the right candidate!
THIS IS A PER DIEM POSITION. SHIFTS WILL VARY INCLUDING DAYS, EVENINGS, NIGHTS AND WEEKENDS. PATIENTS ARE THROUGHOUT THE ORANGE COUNTY AND GREATER LOS ANGELES AREAS.
Job Requirements
Registered Nurse currently licensed in California.
Must have a minimum of one year general nursing experience with current medical, surgical, or critical care experience.
Must demonstrate a commitment to the hospice philosophy of care
Experience in providing services to terminally ill patients preferred.
Must be a licensed driver with a reliable automobile that is insured in accordance with state and/or organization requirements.
